Dickkopf-1

◎ Phase II (anti-DKK1 antibody)
Page last reviewed

Quick Summary

Dickkopf-1 (DKK1) is a secreted Wnt signaling antagonist that binds the LRP5/LRP6 co-receptor with high affinity, blocking canonical Wnt/beta-catenin pathway activation. DKK1 is critical for head induction in vertebrate embryos (Dickkopf means "thick head" in German), and in adults it regulates bone homeostasis, tumor biology, and immune cell function.

Mechanism of Action

  • Binds LRP5/6 third beta-propeller domain via C-terminal cysteine-rich domain-2 (CRD2), sterically blocking Wnt ligand from forming Frizzled-LRP5/6 ternary complex
  • DKK1-LRP5/6 complex is internalized with Kremen1/2, reducing surface LRP5/6 levels and amplifying anti-Wnt effect
  • In bone: DKK1 from osteoblasts suppresses osteoblast differentiation in negative feedback; DKK1 excess from tumor cells drives osteolytic lesions
  • In cancer: DKK1 inhibits anti-tumor immune surveillance by suppressing Wnt-driven NK cell and CTL activation; DKK1 blockade restores immune cell function
  • Embryonic: DKK1 from organizer region antagonizes Wnt at anterior end, allowing head structures to form by preventing caudalizing Wnt signals

Research Findings

  • DKK1 knockout mice die perinatally with severe cranial and limb defects; heterozygous knockouts have increased bone density
  • Multiple myeloma cells secrete high DKK1, suppressing osteoblast activity and driving osteolytic bone disease; anti-DKK1 antibodies restore bone formation in myeloma models
  • BHQ880 (anti-DKK1 antibody): Phase II clinical trial in smoldering myeloma improved bone density vs zoledronic acid alone
  • DKK1 serum levels correlate with osteolytic lesion burden in myeloma, Ewing sarcoma, and breast cancer bone metastases
  • Anti-DKK1 immunotherapy combined with PD-1 blockade enhanced anti-tumor CD8+ T cell responses in preclinical solid tumor models

Research Protocols

  • Recombinant DKK1: 1-100 nM to suppress Wnt3a-stimulated osteoblast differentiation; IC50 ~2-10 nM
  • Myeloma bone disease model: 5T2MM myeloma cells + DKK1 neutralization with 100 mg/kg IP anti-DKK1 antibody weekly in C57Bl/KaLwRij mice
  • Tumor immunology: DKK1-knockout tumor cells implanted in syngeneic mice; NK cell and CTL activity assessment by ELISPOT
  • LRP6 binding assay: recombinant DKK1 displacement of Wnt3a from LRP6 ectodomain at 0.1-100 nM

Interactions

  • Sclerostin: both inhibit LRP5/6 but bind different beta-propeller domains; cooperative suppression of bone Wnt signaling
  • Kremen1 and Kremen2: DKK1 forms ternary complexes with Kremen and LRP5/6, driving LRP endocytosis and amplifying DKK1 effect
  • Anti-DKK1 antibodies (BHQ880, DKN-01): clinical-stage therapeutics for myeloma and advanced cancers

Safety Profile

Recombinant DKK1 is a research tool. Anti-DKK1 antibody therapy: generally well tolerated in Phase I/II trials. Theoretical concern is anabolic bone effects causing hypercalcemia at high doses; not observed clinically at tested doses.
